We will continue to use the 5-copy and switch when inventory of the 4-copy comes available.
During the transition from the 5-copy to the 4-copy hazardous waste manifest form, exporters and importers who continue using the obsolete 5-copy form must enter the consent numbers for their wastes in Item 14 (Special Handling Instructions and Additional Information). When required, exporters should also include their EPA ID number in this field, formatted as “Exporter EPA ID #AAANNNNNNNNN” for clarity.
The EPA considers this approach an effective way to ensure that the same required information is captured on the 5-copy form (and any continuation sheets) as would be recorded on the updated 4-copy form during this transitional period.
